Anybody who wishes to take part in the Whimple History Society Lottery, needs to complete and submit an application form.

There are two options to complete this application process: -
 

1. Complete the below online application form which will automatically be sent to our Treasurer and you will also receive a conformation email with payment details as well an attached copy of your completed application for your records.
    

2. Download an Acrobat pdf copy of the form via the green on-screen button below and print it out accordingly.  This version of the application form is for those who wish to submit a physically completed copy.
